2. Provide location details: Specify the state and address of the incident, ensuring accurate documentation.
3. Include a comprehensive description: Provide a detailed explanation of the incident you want to report, helping to provide context and aid investigations.

The provision of informant details is optional, ensuring a level of anonymity for those who wish to remain undisclosed.
